Well, speaking from personal experience, it was total immersion in an environment where virtually no one spoke English. That is how I learned German and Czech. I realise that this would not suit everyone; so my advice is for each person to find the way that is most effective for them
First of all, a teacher should provide a comfortable learning environment where students can express themselves fully. Secondly, a teacher should plan lessons by keeping in view the needs and levels of his students. These lesson plans must also include some interactive activities.
There are some really great ideas in the question itself actually. I personally think there isn't honestly a "best" way to learn English. We all learn different things differently. But in general, and from personal experience, two main things are helpful: 1. A tutor: to help with explanations and to guide you 2. A practical way to practise what you learn. This may involve living in the country where that language is spoken, or surrounding yourself with that language through Music, Movies, Books etc.

Spend time building a friendship with those who speak it. Learn from them. Listen to them talk, read what they write and mimic them. Language and culture are inseparable. A learner who doesn’t understand culture will not communicate effectively.
