With a big smile and say thank you so very much.
With a huge smile, say: "Thank you so much. You really shouldn't have". However, be careful with intonation as it is all too easy to communicate sarcasm with it. That would really poop the party mood
Depends on how bad it is but any way the gift giver has cared about you by getting you a gift. So you can just say "I appreciate your kindness"

It depends on the giver. Sometimes I accept the gift and try to show my grateful specially when I know that they come from poor family but other times I give back the gift sincerely and mention that this gift may be useful for the other persn
Aways say thank you and if you have a grateful attitude, you won't see it as a bad gift but rather as a gift from someone who cares enough to think of you on a special day. Focus on the person rather than the gift then it won't be difficult to be grateful and sincere.
