I guess it all depends on the level you want to reach and the amount of time you get exposed to the language. The amount of time it will take you to reach the level of English you want is proportional to the amount of time you get exposure to the language. The more you read and listen to English, the faster you'll speak and write.

Yes and no; it depends on how you want to use it. If you're using it for traveling, you only need to know some basic words and phrases. These can be learned quickly. If you want to chat with an English speaking friend, this can take longer. However, you don't need to know everything overnight. Practice what you know and keep adding to it, and you will be fine no matter how much you know or don't know.
It also depends on the student's level and picking power.
It requires time and effort. Greater levels of immersion can help you learn quicker.
