This is an interesting question, it depends on your students. Tutors need to carry out a needs analysis on the first session to see what their students need in order to reach their goals, then plan accordingly.
You could go with the tried and tested by the book way however as a neurodivergent like myself sometimes you need more than just a book. Something like patience and care are needed. No placement test are necessary as you should meet with them and discuss where they are at.
Before anything else, a placement test must be done to see where the students strengths and weaknesses lie. Once known, the teacher can then plan a course of action for the student. However, the student must realise that English is not a subject like Physics or Geography and must put in the time outside lesson time to learn, ie the student cannot just forget about English until the next lesson: languages don't work that way.

Start testIs it even always possible to have your preferred theoretical basis for a lesson, in my humble opinion it would be what the student requires, after completing an online assessment and the result should guide you, even though that is always not the case, as it varies from student to student.
I have clarified notes written the way I understand. As I identify the students needs through interaction, I can clarify for them concepts.
