My grandmother is in excruciating paid, she fell of a chair and tried to balance with her, arm. Now she strained her main arm ligament, she is really in pain.

The best way which is grammatically correct is I am in pain
I am in pain
"I am in pain" would be correct
I'm in pain. My head hurts. My arm is hurting. I'm not certain "pain" is a verb, nor if it lends itself to conjugation.
I am in pain is the correct answer. Pain is a noun, not a verb, so you can't say 'Its paining'.[Note that pain is a verb in with the sense of causing pain to someone.]
